November is National Diabetes Month, and it’s an opportune time to shine a light on a critical area of diabetes care—health equity. At Umemba Health, we’re dedicated to supporting healthcare and public health professionals in delivering care that not only treats the condition but also addresses the systemic disparities that impact patient outcomes. Our new Diabetes Equity Workshop was developed with this exact mission in mind, providing healthcare professionals with the knowledge and tools to create equitable care pathways for individuals and communities affected by diabetes.


The Growing Need for Equity in Diabetes Care

Diabetes affects more than 38 million people in the United States, and a significant portion of these individuals belong to high-priority communities that face unique social, economic, and healthcare challenges. Social Drivers of Health (SDOH)—factors like socioeconomic status, access to care, housing, and education—are deeply intertwined with diabetes outcomes. When healthcare providers understand and address these social determinants, they’re better equipped to break down barriers and bridge gaps in care, making impactful strides toward health equity.


Why Health Equity Matters for Diabetes Prevention and Management

Consider the reality for many patients with diabetes: limited access to resources, compounded by socioeconomic and cultural challenges, can make it difficult to manage their condition effectively. Evidence-based interventions like Diabetes Self-Management Education and Support (DSMES) and the Diabetes Prevention Program (DPP) can be life-changing, but they must be coupled with an understanding of each patient’s unique social context to truly succeed. By addressing the social, cultural, and economic realities impacting patients, we’re moving beyond “one-size-fits-all” approaches to deliver care that resonates and, ultimately, results in better outcomes.

At Umemba Health, our Diabetes Equity Workshop goes beyond conventional training. We emphasize critical areas such as:

  • Understanding Social Drivers of Health and how they intersect with diabetes disparities.
