PROGRAM & PARTNERSHIP DEVELOPMENT
Health equity is our foundation. That's why we help organizations build trusted relationships in the communities they serve, and stand up Community Health Worker programs that hold up over time.
Earn trust before you ask. We help you build community and partnership networks the right way, using our T.R.U.S.T. Framework, so that by the time you reach out, the relationship is halfway built.
When health organizations try to reach communities that have been left out or overlooked, they usually try one or two things; paying for leads, or mass outreach to every community group they can find.
This is especiallly true for communities that have been over-marketed to and historically underserved can quickly recognize transactional outreach. Sustainable partnerships require a different approach.
Five steps, in order. Each one builds on the trust earned in the step before. The is the same method we use with our own partner networks.

Listen first. Learn how a communty already workds, what they care about, who they already trust, and what has let them down before. Go where they already meet, without pitching anything.

Give something real before you ask for anything. Free education, a helpful resource, an introduction, or your own expertise, offered with no strings attached and no expectation of return.

Inside every organization and community, there are one or two people who truly believe in the work. Find them and make their job easier. One trusted champion moves things further any cold outreach campaign will.

Once you have early proof, formalize a small, named group of first partners. Calling them founding partners takes the pressure off and feels exclusive, instead of like a mass, generic ask.

With real case studies, testimonials, and strong champions, you can grow with confidence. This is when things speed up, because you are leading with proof instead of promises.

Starting a CHW program is more than picking a curriculum. It takes roles, workflows, supervision, data, and a plan to pay for it all. Design, build, and sustain a CHW program from the ground up. We help you build the whole thing, and build it to hold up.
01
Decide what your program is, who it serves, and where your CHWs sit, whether that is in a clinic, a community setting, or both.
02
Write clear job descriptions and define what your CHWs do, and just as important, what they do not do.
03
Recruit the right people, welcome them well, and set up the supervision that keeps them supported and growing.
04
Build the day to day workflows and referral paths so your CHWs are a real part of the care team, not an add on.
05
Choose the right measures, set up your documentation, and show the impact your program is making.
06
Plan for the long haul with a funding strategy so your program is still standing after the first grant ends.

The T.R.U.S.T. Framework is a five-step method for building community and partnership networks by earning trust before you ask for anything. The five steps are Tune In, Reciprocate, Uplift Champions, Solidify Founding Partners, and Transcend to Scale. It was created by Umemba Health.
CHW program development is the work of building a complete Community Health Worker program. It covers program design, roles and scope of practice, hiring, onboarding, supervision, workflows, performance measures, and a funding plan for the long term.
Curriculum is the content your CHWs learn. Program development is everything around it: the roles, the workflows, the supervision, the data, and the funding. Many organizations need both.
A Strategy Session is a single call that shows you the framework and your best next step. A VIP Day is a deeper session where you leave with a full phased roadmap for your community network. Partnership Development is a 6 -to 12-month retainer where we walk through all five steps with you in real time, including warm introductions to our own network. Book a discovery session to learn more about each service in detail.
