What is Diabetes Self-Management Education or DSME?

DSME is “the active, ongoing process of facilitating the knowledge, skill, and ability necessary for diabetes self-care.” At Umemba Health, we provide diabetes self-management education and support (DSMES).

How do you benefit from DSMES?

There is considerable evidence linking DSMES participation to positive changes in health behaviors and improved diabetes-related outcomes. Benefits of DSMES participation can include:

  • Improved hemoglobin A1c levels

  • Improved control of blood pressure and cholesterol levels

  • Higher rates of medication adherence (you take your medication better)

  • Fewer or less severe diabetes-related complications

  • Healthier lifestyle behaviors, such as better nutrition, increased physical activity, and use of primary care and preventive services

  • Enhanced self-efficacy (you are more confident in your ability to manage your diabetes)

  • Decreased health care costs, including fewer hospital admissions and readmissions

If you have diabetes, you need a Certified Diabetes Educator (or Certified Diabetes Care and Education Specialist) to help you learn the skills needed to manage your diabetes successfully. The Diabetes Educator will also help you to identify barriers to self-care and create realistic and achievable health-related goals.

Our Virtual Diabetes Education Program meets once a week for 4 weeks. Each class is facilitated by a Certified Diabetes Care and Education Specialist. In addition to weekly education sessions, participants receive weekly check-in's between sessions via a free mobile coaching app. Participants can also schedule coaching sessions during CDE “office hours” for extra guidance and support.
